Working Together

This agreement sets out the terms and conditions of working with Siân Quipp.

1. Coaching and Cognitive Hypnotherapy can be a very positive and powerful experience. We can work together on both work and personal issues. However, because of the individual nature of sessions, no guarantee of results can be given.

2. We set out aims and intentions for each session together. Resources and tasks in between sessions may be recommended to consolidate learning and changes.

3. I draw on my experience and training in the fields of Coaching and Cognitive hypnotherapy to best suit your needs which are very effective for mindset, emotional blocks and goals. We discuss and agree together the best approach for each session.

4. Appointments will take place over Zoom, Teams, phone call or in-person which will be agreed in advance. Contact between our sessions will be reasonable and limited to telephone and email.

5. Please give 24 hours’ notice if you’d like to rearrange an appointment. Any sessions cancelled within 24 hours will incur the full session fee.

6. Any anti-social behaviour will result in the immediate cessation of treatment.

7. Notes will be taken throughout the sessions to aid with the treatment plan and are available to you by request and within 7 days. They will be stored in accordance with GDPR and ICO’s guidance.

8. I will ensure that your confidentiality will be maintained in all but the most exceptional circumstances and all information collected during the sessions will be protected at all times. If you are a danger to yourself or others then I have a duty of care to help you to access further support and we can discuss the best options together - including but not limited to a crisis team, your GP or HR. In an emergency situation your emergency contact may be used. I will remind you of this procedure at the start of the first session.

9. To get the most out of our sessions and to work safely together please ensure you have answered any questions regarding your current wellbeing, your medical history and any medication or drugs you’re taking accurately. It is also your responsibility to inform me of any issues or changes which you believe may be relevant to your treatment.

10. I will suggest alternative provisions for illnesses such as psychosis, hallucinations, suicidal thoughts, borderline personality disorder, bipolar disorder, epilepsy and any other conditions that are contraindicated within cognitive hypnotherapy.
